Conservative approaches may include occlusal splints, physical therapy, and\u00a0<em><strong><a title=\"mayofacial pain release\" href=\"https:\/\/mhnpc.com\/2023\/08\/31\/myofascial-release-for-tmj-pain\/\">mayofacial pain release<\/a><\/strong><\/em>.<\/p>\n<p>But let\u2019s back up and start with some basics.<\/p>\n<h3><strong>What is Central Sensitization?<\/strong><\/h3>\n<p>Central sensitization can be defined as an amplified response of your central nervous system to sensory stimuli and peripheral nociceptive. It may lead to increased pain sensation from noxious stimuli, known as hypersensitivity (allodynia).\u00a0<em><strong><a title=\"Chronic temporomandibular disorders\" href=\"https:\/\/mhnpc.com\/pain-relief\/tmj-disorders\/\">Chronic temporomandibular disorders<\/a><\/strong><\/em>\u00a0are associated with a higher propensity to develop central sensitization.\u00a0<sup>[1]<\/sup><\/p>\n<p>Central sensitization is like your brain\u2019s alarm system becoming overly sensitive. Normally, when you get hurt, your nerves send a pain signal to your brain. With central sensitization, this system gets overactive. It\u2019s like a blaring alarm even when there\u2019s no imminent danger.<\/p>\n<h3><strong>What does Central Sensitization have to do with TMD?<\/strong><\/h3>\n<p>Central sensitization is a central nervous system (CNS) process that can trigger pain hypersensitivity. It can be a factor in\u00a0<span id=\"urn:enhancement-e9a9e861-cc57-46f3-a33c-d57d2b9bae33\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0associated with temporomandibular disorders (<span id=\"urn:enhancement-8b7e59db-b782-4700-bd90-cb8de4b53391\" class=\"textannotation disambiguated wl-thing\">TMD<\/span>), a group of conditions that affect the temporomandibular joint (TMJ), masticatory muscles, and trigeminal nerve.<\/p>\n<p>Chronic TMDs are largely characterized by\u00a0<span id=\"urn:enhancement-f4d4171b-6ce6-402c-b545-fa46e15986a8\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>, which is also a characteristic of CSS.<\/p>\n<h3><strong>Central sensitization relationship to TMD explained<\/strong><\/h3>\n<p>When persistent and intense nociceptive sensitized central nervous system pain signals are emitted, the patient becomes more reactive. They often experience more pain with less provocation. Pain management practitioners who know pain signals can best determine the origins of orofacial pain. A deep understanding of the trigeminal system and mechanisms differentiating pain types and causes helps clinicians treat the underlying cause rather than just reported symptoms.<\/p>\n<h3><strong>Central sensitization can cause the following symptoms:<\/strong><\/h3>\n<ol>\n<li><strong>Hypersensitivity:<\/strong>\u00a0Normal pain feels much worse than it should.<\/li>\n<li><strong>Allodynia:<\/strong>\u00a0Something that normally wouldn\u2019t hurt at all, like a light touch to your\u00a0<span id=\"urn:enhancement-b8219388-40ee-462e-8929-65772bd035dc\" class=\"textannotation disambiguated wl-thing\">jaw<\/span>, starts causing pain.<\/li>\n<li><strong>Altered Pain Patterns:<\/strong>\u00a0Your pain might last longer, spread to neighboring areas, spike, or feel worse over time.<\/li>\n<li>Referred pain.\u00a0<sup>[2]<\/sup><\/li>\n<li><strong>Pressure hyperalgesia:<\/strong>\u00a0An extreme response to pain. Hyperalgesia may happen if there is damage to your nerves or chemical changes to the nerve pathways involved in sensing pain. This may be caused by\u00a0<em><strong><a title=\"jaw tissue injury\" href=\"https:\/\/mhnpc.com\/2022\/08\/29\/pulled-jaw-muscle\/\">jaw tissue injury<\/a><\/strong><\/em>\u00a0or inflammation or by taking certain drugs, such as\u00a0<em><strong><a title=\"taking certain drugs, such as SSRIs and opioids, for chronic pain\" href=\"https:\/\/mhnpc.com\/2022\/04\/27\/chronic-pain-providers-role-in-curbing-the-opioid-crisis\/\">SSRIs and opioids for chronic pain<\/a><\/strong><\/em>.<\/li>\n<li><strong>Increased temporal pain:<\/strong>\u00a0Pressure may be felt around the temples and can feel like a dull, throbbing headache, stabbing, or burning sensation.\u00a0<sup>[3]<\/sup><\/li>\n<li><strong>Masseter muscle pain:<\/strong>\u00a0Your masseter muscle is located on your face from the cheekbone to the mandible on both sides of the face. Its function is to help elevate and protrude the lower\u00a0<span id=\"urn:enhancement-8bb7e764-0e2e-4d21-b4b8-c435936bb014\" class=\"textannotation disambiguated wl-thing\">jaw<\/span>. It also plays a role in stabilizing your temporomandibular joint (TMJ) when you clench your teeth.<\/li>\n<\/ol>\n<p>Think of it like a sunburn. Having one makes your skin extra sensitive. With central sensitization, it\u2019s like your nervous system feels \u201csunburnt.\u201d Your brain\u2019s pain system may be stuck on high alert, making even minor triggers feel much worse. It occurs when the disc is displaced in the closed-mouth position and becomes more normal in the open-mouth position. A chronic condition can be avoided if diagnosis and treatment began at this stage.<\/p>\n<h3><strong>What is the Diagnosis Process for Central Sensitization in Temporomandibular Disorders?<\/strong><\/h3>\n<h3><strong>Its diagnosis is mainly clinical; diagnostic criteria may include the following:<\/strong><\/h3>\n<ul>\n<li>Ruling out other conditions and reviewing the patient\u2019s medical history.<\/li>\n<li>An investigation of central and peripheral factors affecting pressure pain thresholds of the person\u2019s\u00a0<span id=\"urn:enhancement-24bdfc9f-66f2-4df1-91f5-69a3293a77aa\" class=\"textannotation disambiguated wl-thing\">jaw<\/span>\u00a0muscles.<\/li>\n<li>Evaluate if there was a peripheral injury and\/or dysfunction.<\/li>\n<li>Assess if the\u00a0<span id=\"urn:enhancement-22890252-6964-4168-b03e-fb49befa133e\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0has extended beyond a normal healing time.<\/li>\n<li>Assess how your stomatognathic muscles work while at rest and while functioning.<\/li>\n<li>Consideration of a psychological component. It is responsible for many sensory functions, such as your ability to chew, swallow, breathe, speak, yawn, smile, and sing. A strong link exists between chewing functional limitations and the progression of\u00a0<span id=\"urn:enhancement-e3d73d6b-71e2-43cd-98e7-296476d95975\" class=\"textannotation disambiguated wl-thing\">TMD<\/span>.<\/p>\n<p>Therefore, your central sensitization could represent the basis of\u00a0<span id=\"urn:enhancement-c1a55029-d7c3-4bb1-ab93-602a19d202f6\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0if you are affected by\u00a0<span id=\"urn:enhancement-11c1a43a-b0fa-401c-8bbf-090d854404d4\" class=\"textannotation disambiguated wl-thing\">TMD<\/span>.<\/p>\n<p>Scientific literature demonstrates that central sensitization may have a role not only in the pathophysiology of\u00a0<span id=\"urn:enhancement-ba1be351-d6aa-465c-a460-8080bdbc2bb5\" class=\"textannotation disambiguated wl-thing\">TMD<\/span>\u00a0but also in several\u00a0<span id=\"urn:enhancement-ee3f1780-8d63-4819-89e0-1b345ae06746\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0conditions, including:\u00a0<em><strong><a title=\"The relationship between TMD and fibromyalgia\" href=\"https:\/\/mhnpc.com\/2021\/07\/13\/fibromyalgia-tmd-relationship\/\">fibromyalgia<\/a><\/strong><\/em>, migraine,\u00a0<em><strong><a title=\"tension-type headache\" href=\"https:\/\/mhnpc.com\/2020\/04\/16\/alleviating-tension-headaches\/\">tension-type headache<\/a><\/strong><\/em>, and chronic fatigue syndrome.<\/p>\n<h3><strong>What is nociplastic pain?<\/strong><\/h3>\n<p>Nociplastic pain is one type of\u00a0<span id=\"urn:enhancement-c51eb359-6a59-4e8f-9f25-2e20765fe1fe\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0that may happen when pain-related sensory pathways in the central nervous system (CNS) and periphery are altered, causing increased sensitivity.\u00a0<em><strong><a title=\"Trigeminal neuralgia\" href=\"https:\/\/mhnpc.com\/2023\/07\/31\/trigeminal-neuralgias-association-with-tmj-pain\/\">Trigeminal neuralgia<\/a><\/strong><\/em>, peripheral nerve injury, and painful temporomandibular disorders (TMDs) are\u00a0<em><strong><a title=\"chronic orofacial pain conditions\" href=\"https:\/\/mhnpc.com\/about\/disorders-we-treat\/orofacial-pain\/\">chronic orofacial pain conditions<\/a><\/strong><\/em>\u00a0that can be considered chronic peripheral neuropathic pain.<\/p>\n<p>This is very similar to central sensitivity or peripheral pain.\u00a0<span id=\"urn:enhancement-b80aebd4-df56-4749-82d2-2f0f219cc5cd\" class=\"textannotation disambiguated wl-thing\">TMD<\/span>-related\u00a0<span id=\"urn:enhancement-b3bacafa-ada5-4512-8ccd-8b634901c16f\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0has been primarily caused by peripheral factors, such as chronic inflammation of the TMJ. Eight disorders have been identified as relating to central sensitization. Among them are chronic fatigue syndrome, fibromyalgia, temporomandibular disorder, migraine or tension headache, and non-specific emotional disorders that include anxiety or panic attacks and depression.\u00a0<sup>[4]<\/sup><\/p>\n<p>Neuroplasticity refers to brain structures and sensory pathways in your central nervous system that can adapt and be altered in response to sensory input, sensations, and traumatic events. This may include a blow to the\u00a0<span id=\"urn:enhancement-2e0dc11d-6716-43e2-80e4-ddb83dd39ddf\" class=\"textannotation disambiguated wl-thing\">jaw<\/span>,\u00a0<em><strong><a title=\"sports injury to the head\" href=\"https:\/\/mhnpc.com\/2023\/06\/30\/sports-related-injuries-and-tmj\/\">sports injury to the head<\/a><\/strong><\/em>, or whiplash that may trigger\u00a0<em><strong><a title=\"TMJ-related neck pain\" href=\"https:\/\/mhnpc.com\/2024\/02\/22\/tmj-neck-pain\/\">TMJ-related neck pain<\/a><\/strong><\/em>.<\/p>\n<blockquote><p>\u201cNociplastic pain is a type of pain that can describe chronic temporomandibular pain disorders (TMDs). Nociplastic pain is a broad term that can include conditions that cause increased processing of pain signals or decreased pain inhibition. TMDs are a type of temporomandibular joint (TMJ) disorder that can cause pain in the\u00a0<span id=\"urn:enhancement-01281ff1-0048-46a2-a5a8-48c27d5eff63\" class=\"textannotation disambiguated wl-thing\">jaw<\/span>\u00a0joint and muscles that control\u00a0<span id=\"urn:enhancement-16fe688a-d9b7-4d32-826e-b2ce889153d9\" class=\"textannotation disambiguated wl-thing\">jaw<\/span>\u00a0movement. In some\u00a0<span id=\"urn:enhancement-ae2d194e-269b-4fb0-a0e8-bc926ac45270\" class=\"textannotation disambiguated wl-thing\">TMD<\/span>\u00a0patients, nociplastic pain can be a better description of the clinical appearance of altered nociception.\u201d \u2013 NIH: Current understanding of nociplastic pain\u00a0<sup>[4]<\/sup><\/p><\/blockquote>\n<h3><strong>Evidence for Central Sensitization in TMD<\/strong><\/h3>\n<p>Studies suggest that therapeutic strategies for managing central sensitization in\u00a0<span id=\"urn:enhancement-4159244f-b7b5-4796-990e-8ce192c18c42\" class=\"textannotation disambiguated wl-thing\">TMD<\/span>\u00a0pain can be divided into two groups.<sup>[2]<\/sup><\/p>\n<ol>\n<li>Bottom-up (Example: tissue-based impairment treatments).<\/li>\n<li>Top-down interventions (Example: strategies targeting the central nervous system).<\/li>\n<\/ol>\n<p>A bottom-up strategy could include of joint, soft tissue-, and nerve-targeting interventions, such as trigger point injections. A top-down strategy will more likely include physical therapy and pain education. Peripheral sensitisation decreases the depolarisation threshold so that normal stimuli are perceived as painful. The persistence of peripheral pain input can lead to central sensitisation, resulting in increased excitability of central pain pathways. Central sensitisation is the physiological hallmark of persistent pain syndromes and is responsible for the clinical symptoms of hyperalgesia and allodynia.
