This is propelling the urgency for\u00a0<span id=\"urn:enhancement-74d12126-ec40-4ee9-b029-3502ac05d350\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0providers to teach patients\u00a0<strong><a title=\"how to deal with living in an ongoing painful state without drugs\" href=\"https:\/\/mhnpc.com\/about\/treating-preventing-chronic-pain\/\">how to deal with living in an ongoing painful state without drugs<\/a><\/strong>.<\/p>\n<p>The Minnesota Head &amp;\u00a0<span id=\"urn:enhancement-6de30785-04e3-4607-953b-40514e3fb7f3\" class=\"textannotation disambiguated wl-thing\">Neck Pain<\/span>\u00a0Clinic first wrote an article about our concerns in August 2016. COVID, reports of war, avian flu worries,\u00a0<strong><a title=\"loneliness, anxiety, and depression\" href=\"https:\/\/mhnpc.com\/2020\/05\/19\/pain-anxiety-depression\/\">loneliness, anxiety, and depression<\/a><\/strong>\u00a0are pushing the opioid crisis into focus. It\u2019s now clear that the crisis resides predominantly in the streets and that it will take a united effort to curtail it. Constraining opioid treatment within pain clinics alone is unlikely to suffice.<\/p>\n<p>Thankfully, better medical awareness of this health crisis and new CDC guidelines are both emerging.<\/p>\n<h3 id=\"cdc-addresses-urgency-for-less-reliance-on-drugs-to-treat-pain\"><strong>CDC Addresses Urgency for Less Reliance on Drugs to Treat Pain<\/strong><\/h3>\n<p><strong>Cautious use of opioids is recommended for the treatment of patients with chronic pain.<\/strong><\/p>\n<p>A person\u2019s likelihood of developing an\u00a0<span id=\"urn:enhancement-5288a1fb-72e9-4cc0-93fc-ce602367ed03\" class=\"textannotation disambiguated wl-thing\">opioid use disorder<\/span>\u00a0stems from multiple factors. It includes the length of time a person is prescribed to take opioids for acute pain, and how long that individual continues taking opioids (whether as prescribed, or misused). Awareness of the extent of this issue has become a public health crisis and its devastating consequences have increased due to CDC concerns.<\/p>\n<p>The CDC recognizes that patients with pain deserve safe and effective pain management. It states, \u201cPain, particularly chronic pain, can lead to impaired physical functioning, poor mental health, and reduced quality of life. It contributes to substantial morbidity and mortality in the United States each year.\u00a0<strong><em>Chronic pain is the leading cause of disability in the United States<\/em><\/strong>\u00a0and the economic costs are staggering\u2014$560 to $635 billion dollars annually.\u201d<\/p>\n<p>It expresses a need for new guidelines that further embrace non-invasive, non-pharmacological and non-opioid pharmaceutical treatment of\u00a0<span id=\"urn:enhancement-fad7913b-0b81-4bb6-8da9-aeb0c3ce5b8d\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>. It recognizes that Opioids have been too often prescribed in response to a\u00a0<strong><a title=\"jaw tissue injury\" href=\"https:\/\/mhnpc.com\/pain-relief\/tmj-disorders\/\">jaw tissue injury<\/a><\/strong>, or other trauma or surgery. One thing that is clear, the CDC is highlighting of need for\u00a0<span id=\"urn:enhancement-1d7173dc-b6d3-41a1-9c1a-00a651185db8\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0care providers\u2019 clinical judgment when evaluating drug perscriptions for acute pain care.<\/p>\n<p>When the CDC wrote new prescribing guidelines in 2016, the intent was to help doctors make better decisions. The document emphasized the risk of opioid\u00a0<span id=\"urn:enhancement-2451b69f-19e2-46d5-9aba-185fbcb6ed7a\" class=\"textannotation disambiguated wl-thing\">addiction<\/span>, and advised prescribing patients the lowest effective dose. However, our rising crisis puts pressure on\u00a0<span id=\"urn:enhancement-b890509a-f18c-420e-a270-25d732e22882\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0providers to do more for their patients. Individuals given an opioids prescription need to receive updated and factual information on possible side effects, other risks, and monitoring for safety.<\/p>\n<h3 id=\"fda-seeks-non-addictive-alternatives-to-opioids-for-acute-pain-management\"><strong>FDA Seeks Non-Addictive Alternatives to Opioids for Acute Pain Management<\/strong><\/h3>\n<p><strong>February 9, 2022, FDA press announcement highlights:<\/strong><\/p>\n<p>The U.S. Food and Drug Administration took a renewed stance aimed at\u00a0<strong><a title=\"fostering the development of non-addictive alternatives to opioids to manage acute pain\" href=\"https:\/\/www.fda.gov\/regulatory-information\/search-fda-guidance-documents\/development-non-opioid-analgesics-acute-pain-draft-guidance-industry\" target=\"_blank\" rel=\"noopener noreferrer\">fostering the development of non-addictive alternatives to opioids to manage acute pain<\/a><\/strong>. It also hopes to decrease exposure to opioids and prevent new addictions. The agency issued draft guidance for participants to provide recommendations to companies developing non-opioid analgesics for acute pain lasting up to 30 days.<\/p>\n<blockquote><p>\u201cPrescribed appropriately, opioid analgesics are an important part of acute pain management. However, even at prescribed doses, they pose a risk for\u00a0<span id=\"urn:enhancement-eff4d107-ea09-4fc0-bf0e-7de935e9e6ce\" class=\"textannotation disambiguated wl-thing\">addiction<\/span>, misuse, abuse, or overdose that may result in death. A non-opioid analgesic for acute pain that completely eliminates or significantly reduces the need for an opioid could have a major public health impact by alleviating patient suffering from acute pain while mitigating the risks associated with using an opioid.\u201d<\/p><\/blockquote>\n<p>The Opioid Epidemic is \u201ca public health crisis that has killed half a million Americans and continues to claim tens of thousands of lives every year.\u201d So, how bad is it, really? Let\u2019s look at more statistics.<\/p>\n<h3 id=\"us-opioid-use-for-chronic-pain-and-death-statistics\"><strong>US Opioid Use for Chronic Pain and Death Statistics<\/strong><\/h3>\n<ul>\n<li>\u201cDeaths from synthetic opioids are alarmingly high and trending upward. Over the past 12 months, the United States eclipsed 100,000 deaths from illegal drug overdoses for the first time in its history, with more than two-thirds of those deaths caused by illegally produced synthetic opioids.\u201d \u2013 David Luckey, MBA, MS<\/li>\n<li>Roughly 21- 29 percent of patients prescribed opioids for\u00a0<span id=\"urn:enhancement-bfdc787a-c4df-4a51-858d-0737505726e6\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0misuse them.\u00a0<sup>[1]<\/sup><\/li>\n<li>Approximately 25 million US residents have moderate to severe\u00a0<span id=\"urn:enhancement-0bbe73e4-118a-4a8c-96ec-e706756afdea\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>, and an estimated 5 million to 8 million of them have used opioids for long-term management, according to National Institutes of Health data.<\/li>\n<li>Pain is the primary reason Americans are on disability and its societal costs were estimated in 2014 at between $560 and $630 billion annually as a result of missed workdays and medical expenses.\u00a0<sup>[2]<\/sup><\/li>\n<li>In 2019, nearly 50,000 people in the United States died from opioid-involved overdoses.\u00a0<sup>[2]<\/sup><\/li>\n<li>The United States hit 100,000 overdose deaths in a 12-month period for the first time since such data were recorded. Pain care providers\u2019 strategies and treatment options are predicted to reflect possible FDA policy changes on both a regional and national level. \u2013 Bradley D. Stein, MD, PhD, MPH, the director of the Opioid Policies, Tools and Information Center at the RAND Corp., in Pittsburgh.<\/li>\n<li>Roughly 21 to 29 percent of patients prescribed opioids for\u00a0<span id=\"urn:enhancement-2bc1b9ff-c033-4d67-8711-1597943df698\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0misuse them.\u00a0<sup>[3].<\/sup><\/li>\n<li>Between 8 and 12 percent of people using an opioid for\u00a0<span id=\"urn:enhancement-d149491b-86f6-4c69-bdec-8d317aec4993\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0develop an\u00a0<span id=\"urn:enhancement-65066c17-3917-498b-b53d-ffd1fc6d4fa5\" class=\"textannotation disambiguated wl-thing\">opioid use disorder<\/span>.\u00a0<sup>[3].<\/sup><\/li>\n<li>Morbidity, mortality, and financial costs of inadequate treatment of the 18 million Americans with moderate to severe\u00a0<span id=\"urn:enhancement-b50eeef3-eec7-4c9f-b5f9-d56c453d3486\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>\u00a0are high.<\/li>\n<li>Overdose deaths involving any opioid increased by 44% from 2,939 in 2019 to 4,233 in 2020 at an average of nearly 12 resident deaths every day during 2020.\u00a0<sup>[4]<\/sup><\/li>\n<li>Nationally, overdose deaths were the highest in history with over 91,000 drug-involved overdose deaths and over 68,000 opioid-involved overdose deaths in 2020.\u00a0<sup>[4]<\/sup><\/li>\n<\/ul>\n<p>Since our clinics are based in Minnesota, here are some local statistics.<\/p>\n<h3 id=\"minnesota-opioid-use-for-chronic-pain-and-death-statistics\"><strong>Minnesota Opioid Use for Chronic Pain and Death Statistics<\/strong><\/h3>\n<p><strong>Minnesota, like many other states, is currently experiencing opioid overdose deaths at an epidemic level.<\/strong><\/p>\n<p><span id=\"urn:enhancement-d30a234e-4ab9-484d-aea7-3c9b5c19f521\" class=\"textannotation disambiguated wl-thing\">Chronic pain<\/span>\u00a0providers have a role in clinical oversite so that drug overdose is more preventable. Below are current\u00a0<a title=\"MN Department of Health opioid death-related statistics\" href=\"https:\/\/www.health.state.mn.us\/opioiddashboard\" target=\"_blank\" rel=\"noopener noreferrer\">MN Department of Health opioid death-related statistics<\/a>.<\/p>\n<ul>\n<li>In 2015 Minnesota had over 10,000 treatment admissions for heroin (6485) and other<br \/>\nopiates (3,777). Of these, 34.4% were admitted into OTPs and 65.6% were admitted into abstinence-based treatment programs.<\/li>\n<li>A 2015 analysis of Minnesota state death certificate data found 402 opioid-related deaths in 2016, up from 344.<\/li>\n<li>In 2019, synthetic opioids were involved in the highest percentage of opioid overdose deaths. The state of MN reported synthetic opioids at 55.38, commonly-prescribed opioids at 25.63%, and Heroin at 19.00%.<\/li>\n<li>The percentage of Minnesota students using prescription pain medications without a prescription or differently than how a doctor instructed was higher in 2019 than in 2013.<\/li>\n<li>The amount of methamphetamine seized has increased since 2011, indicating that the supply of methamphetamine has increased in the state of Minnesota.<\/li>\n<li>In 2020, individuals between 15-34 years of age had the greatest number of ER visits for opioid-involved overdoses. For a breakdown, 1522 cases were reported for residents ages 25-34; and 1126 for residents between the ages of 15 and 24.<\/li>\n<li>Males had more ER visits than females for opioid-involved overdoses in 2020. 66.12% was attributed to males and 33.88% to females.<\/li>\n<li>Minnesota prescription-related deaths are second only to alcohol.<\/li>\n<li>Preliminary reports show 654 opioid-involved deaths in Minnesota in 2020, a 59% increase from 2019<\/li>\n<li>Approximately 59% of all drug overdose deaths are attributed to opioids, and 18% of those deaths are alcohol-related. Significant increases in the proportion of OOD were found between 2019 and 2020, according to Jessica K. It ties into<a href=\"https:\/\/mhnpc.com\/2019\/02\/20\/suicide-prevention-and-awareness-provided-by-pain-management\/\">\u00a0Suicide Awareness concerns when prescribing medications<\/a>.<\/p>\n<h3 id=\"safe-effective-non-addictive-chronic-pain-management-strategies\"><strong>Safe, Effective, Non-addictive Chronic Pain Management Strategies<\/strong><\/h3>\n<p>Responding to the opioid crisis, the U.S. Department of Health and Human Services (HHS) is focusing efforts on providing support for cutting-edge research on pain and\u00a0<span id=\"urn:enhancement-fa3b9847-1d5c-48fe-afd5-a899ab763da0\" class=\"textannotation disambiguated wl-thing\">addiction<\/span>, as well as advancing better practices for pain management. In Minnesota, our pain clinics focus on reducing overdose by providing easy access to buprenorphine, building interdisciplinary safety plans with our pain patients, and caring for their long-term recovery without any drug dependencies evolving.<\/p>\n<p>Opioid prescription directives have swung over the last five years from the liberal treatment of pain to rigorous restriction to the slow rise in opioid-related deaths when treating\u00a0<span id=\"urn:enhancement-a3c463e0-925a-4e01-a1ea-db48f44a4f78\" class=\"textannotation disambiguated wl-thing\">chronic pain<\/span>.<\/p>\n<blockquote><p>\u201cOpioid misuse and abuse remain a serious public health crisis facing the country. Preventing new\u00a0<span id=\"urn:enhancement-61659911-7509-4827-abf7-cbcbb2671feb\" class=\"textannotation disambiguated wl-thing\">addiction<\/span>\u00a0through fostering the development of novel non-opioid analgesics is an important priority for the FDA. The guidance reinforces the agency\u2019s commitment to confront opioid misuse, abuse, and\u00a0<span id=\"urn:enhancement-98987080-6b4a-441f-9cee-a3ea94ede71e\" class=\"textannotation disambiguated wl-thing\">addiction<\/span> by taking steps to help those with acute pain get access to improved non-opioid treatment alternatives.\u201d \u2013 Patrizia Cavazzoni, M.D., director of the FDA\u2019s Center for Drug Evaluation and Research.<\/p><\/blockquote>\n<section>\n<h3 id=\"chronic-pain-providers-reporting-data-by-state\"><strong>Chronic Pain Providers and Opioid Death Reporting Data by State<\/strong><\/h3>\n<p><strong>Table 1. Model results for each of the ten drug outcomes of interest are presented below. Minnesota has done well in reporting and tracking \u2013 data that is helpful to\u00a0<span id=\"urn:enhancement-08764464-3fcc-49d4-9fd3-294b70c39279\" class=\"textannotation disambiguated It\u2019s good to see increased awareness and commitment to retaining pain patient engagement, providing better self-care boundaries, and minimizing prescription risks.<\/p>\n<p>As the CDC and other groups take a hard look at policy that regulates opioid prescriptions to pain patients.
